 I like option 1, but I'm worried there might be cases where it's
 inappropriate. Can anyone think of any?

 Option 2 would be acceptable.

 Option 3 seems like overkill to me.

 I don't like options 4 and 5 because they require writing settings to
 disk. Too many things can go wrong with that (disk full, partition mounted
 read-only, no permissions, etc.), and I feel the situation is more of a
 temporary error than something that needs to be recalled and fixed later.

 Option 6 is unacceptable. 8)

 Option 7:  Only save the disabled status to disk when changed explicitly
 by the user.
