 By "safety" I mean preventing errors in general, not just data loss
 errors. Errors can cause loss of time, money, face, reputation, and/or
 satisfaction. In this case, wasted time is the main concern.

 I recognize that ''t''(A) is hard to estimate, because it has a wide
 range. In the best case, you see the two windows before you've entered any
 data, you recognize that they're independent "Add Account" windows, you
 understand that dismissing one will not dismiss the other, and you dismiss
 one of them. As you say, that might take as little as a second or two. In
 the worst case, the windows appear exactly on top of each other (as they
 do in Ubuntu's Compiz, ''contra'' my attached screenshot), you don't
 notice the extra button in your window list (since it's not the locus of
 attention), you take a minute to fill in all the account settings, when
 you click "Save" it appears as if the window has been blanked out, you
 take five seconds or so to think about this, you decide to try entering
 the account details again, when you're finished you're confronted with an
 error message, it doesn't tell you how to fix the problem, you think about
 it some more, and eventually you decide to give up, but on clicking
 "Cancel" you see that the account has somehow been set up already. That
 might take two minutes or more. I think 30 seconds is a reasonable
 ballpark for the average, but even if it isn't, I'm also confident that
 P(A) >> P(I). :-)
