 The problem seems to be related to blank (server-generated) resources.

 When the user reconnects, the MUC sends an unavailable presence from the
 previous resource (see the `jid` attribute on `<item>`). My gut instinct
 is, if this is isn't a server issue, we should be making sure not to call
 `serv_got_chat_left` if the `real_jid` in the unavailable presence doesn't
 match our JID.
