 Pretty easy one to reproduce:
  1. Insert smiley from toolbar.
  1. Press Ctrl-Z. Smiley disappears.
  1. Insert smiley from toolbar.
  1. Press Ctrl-Z. Smiley disappears.
  1. Insert smiley from toolbar.
  1. Press Ctrl-Z. Smiley does not go away.
