 Does this happen while you are consistently in a room or does it only
 happen if you part and rejoin the room (even automatically)? Does this
 happen for people who are also SOPs or some other higher-than-op type?

 A recent conversation in #pidgin caused me to look into the irc code for
 this a bit and I noticed that (due to how IRC /names responses work) a
 user with a level higher than op will not correctly be listed as an op
 (and given the icon) if they are already in the room when you join it.
 They will, however, be listed correctly (and given the icon) if they join
 after you are in the room (i.e. they get a mode +Xo set on them).
