 After months of living with this bug I can add two symptoms that trigger
 the problem:

 (1) Only happens when multiple monitors are connected. I'm using a laptop
 screen with an external LCD connected via a port replicator.

 (2) Windows Logon screen is displayed. Only reproducing the problem when
 the screen is turned off due to inactivity as defined in system Power
 Options. Then by moving the mouse screen is activated and logon screen is
 displayed. Once logged in, the problem reproduces (not always but quite
 frequent). The same obviously applies when the system is coming out of
 sleep/hibernate when again I have to use the logon screen as defined in
 system security policies.

 Both (1) and (2) have to be true otherwise I never see the problem. If the
 external LCD is not connected then it's never reproduced.
