This seems to happen for several different reasons, but an easy one to run into is account deactivation, particularly if you signed up for a Yahoo! account on the Yahoo! Mail site instead of the main Yahoo! site. If you don't log into Yahoo! Mail for four months, your mail account is deactivated; this seems to be at least partially related to the login failures error.
