 Do we want to think about putting username/domain/password entry fields
 and the protocol dropdown box directly in the Accounts dialog when no
 accounts exist (or possibly always)? This would allow for simpler account
 creation when the 'extra' fields aren't needed, which would solve some of
 the complaints we've gotten about how the Add Account dialog is confusing
 (because of things like Local Alias).
