 For something like patch to reasonably be applied, a bunch of changes are
 needed.

 For one, none of these changes are necessary when the docs are generated
 for embedding in Trac (which is the default use case).  Any formatting
 should only be applied when generating the documentation for "external"
 consumption.

 Additionally, there is a ton of unrelated formatting being applied (all
 the trac styling) - only the formatting that is specifically needed for
 the docs should be in the header.
